Pledge Form
Pledge forms and money must be turned in no later than 5/7/11 between 9-10:30 am during the on-site registration and sing in. The sign in booth will be located in the Pharaoh Theatre parking lot at the intersection of Maple and Liberty Streets on the Independence Square. Pledge forms and money can also be turned in prior to the Walk at the Independence Animal Shelter at 875 Vista Ave; Independence, MO 64056.
While it is not required, walkers are encouraged to pre-register and get sponsors to pledge money in support of their walk. Ask family, friends and businesses to sponsor your walk to promote spaying and neutering. If you are sponsored by a business, that business can be recognized on the event's website. Email jpolston@indepmo.org to notify us of business sponsors and their logo or business information will be added to the website. We will need the Business Name, a contact person and a phone number to contact them for their logo. Business sponsors to be recognized on the website should consider sponsoring at a minimum $25 pledge. Business sponsors will be recognized the day of the event as well.
